You will receive a payment receipt by email. Click on the button below to proceed with your payment. You'll be taken to a secure page on PayPal where you'll be prompted to enter your payment information (be sure to enter the payment amount).
Note: If you plan to seek reimbursement from your extended health insurance company, your online receipt verifies your payment, but otherwise provides insufficient information for your insurance company to process the claim. When you submit your claim, therefore, along with your receipt you will need to include the invoice you received from me which verifies the provider's information, the date of service, the specific type of service, and the person who received the service.
